As we all know, insurance can be tricky, and eliminating unnecessary paperwork is appreciated by everyone! Some insurance plans require a referral for Medical Nutrition Therapy, while others do not. Feel free to send the referral form below OR direct your patients to our website, where they can schedule an Intro Call themselves. We’d be happy to contact your office if we learn that a referral is needed after verifying insurance.

SENDING A REFERRAL

Follow these 2 steps:

  1. Download the REFERRAL FORM‍ ‍

  2. Fax the completed referral form to (971) 366-0137

    Faxes are received through Kalix - a secure HIPAA compliant EMR

What Happens Next?

Ashley will personally review each referral and contact your patient within 3 business days to schedule an appointment and discuss insurance coverage.
